P31: OPPS Module — outpatient payment system parity with PFS
Add outpatient prospective payment system (OPPS) as a peer module to PFS. Module skeleton with config and table schemas, APC weight file ingestion, payment calculator, skin substitute payment history, express functions with pipe module, and CLI/API routes.

All 6 issues closed. OPPS module (src/opps/) follows the same skinny-package pattern as PFS: init.py, config section in stack.toml, and Pydantic table schemas for APC weight data. APC weight file ingestion handles both annual and quarterly CMS updates with proper effective-date tracking. Payment calculator computes APC-based reimbursement amounts. Skin substitute payment history tracks the transition from APC-based to flat-rate payment — central to the P29 skin substitutes analysis. Narwhals express functions and pipe module wired with full schema validation (189 total pipeline steps across all modules). CLI and API routes added for running OPPS pipelines programmatically.
