P15: CI Plugins

Replace hand-rolled CI shell commands with official Woodpecker plugins, add api image to pipelines, wire diag failure reporter.

All 11 issues closed. Replaced raw docker build/push with the buildx plugin across deploy.yml, rebuild-all.yml, and infra-ci.yml — faster layer-cached builds with multi-platform support. Replaced raw trivy invocations with plugin-trivy. Expanded S3 plugin for badge uploads and all scan artifacts. Wired api.diag failure reporter into all pipelines for automatic Gitea issue creation on CI failures. Added Gitea Comment plugin for PR feedback, Gitea Release plugin for tag releases, and Git Push plugin for coverage badge commits. Added fhirworx/api image to all pipelines. Documentation updated.
