notebook: palliative_care_rfi.py — payment current state + CY2027 proposals for ACP / care management / APCM code families #647

New marimo notebook notebooks/palliative_care_rfi.py quantifying the code families the RFI names, every figure live from lake/registries (repo convention), citations via bib.frlink/cfrlink (fr_md/cfr_md pattern from the CY2027 notebook, #642):

  • ACP 99497-99498: RVU + national payment trajectory (pfs.rvu, pfs.rules), CY2027 proposed values (pfs.rvu_proposed CMS-1848-P)
  • Care management families (CCM/complex CCM/PCM/TCM G-codes and CPT): same treatment; note which are also MSSP designated primary-care attribution codes (cms.primary_care_service_code)
  • APCM G0556–G0558: current pricing + anything CMS-1848-P proposes
  • Year columns through conf.display.plain_years at display sites (#643)

Output feeds P44: one exhibit per RFI question the lake can inform, labeled with which question it supports.
