Component 5. Dedicated loader (src/pfs/nprm.py) reading the NPRM Addendum B from bib storage into pfs.rvu_proposed (pfs.rvu shape + cms_rule_id), delete-and-reload per rule id, wired into dev/scripts/ingest_pfs.py --nprm under duckdb_batch, replica + lake publish, rows logged to cms.ingest_log.

Merged to main in 53b97b8 (branch p36-cy2026-rule, 17 commits). Done in 6563a54 + c0ad6f8 + bc248ea. src/pfs/nprm.py loads the NPRM Addendum B from bib storage into pfs.rvu_proposed: 14,169 rows (CMS-1832-P), delete-and-reload idempotent, in aco.duckdb + read replica + DuckLake. Real-file quirks (padded cells, NA markers, footnote rows) handled and mutation-tested. cms.ingest_log provenance rows carry source_file, sha256, 90 FR 32352, pincite 2KVJ2HKX. Lake publish extended to cms schema. Follow-up #607: column-name alignment with pfs.rvu.
