Objective

Build utilization dataset from available claims data.

Tasks

  • Filter medical claims to skin substitute HCPCS codes
  • Extract: claim_id, person_id, service_date, hcpcs_code, units, paid_amount, rendering_npi, facility_npi, place_of_service, diagnosis codes
  • Link to eligibility for beneficiary demographics (age, gender, state, dual status)
  • Link to provider data (specialty, practice address, NPI taxonomy)
  • Build: skin_subs.claims, skin_subs.providers, skin_subs.beneficiaries
  • Both synthetic (for development) and real CCLF data (for analysis)
  • Compute utilization metrics: claims per beneficiary, units per claim, spend per beneficiary, by provider/setting/geography

Pre-extraction findings:

Surveyed core.medical_claim (167,995 rows). Found:

  • Q4081 (10 lines) and Q4018 (1 line) — both are ESRD drugs, NOT skin substitutes
  • Zero skin substitute product codes (Q4100+) in the synthetic claims data
  • Zero application codes (15271-15278) found either

This is expected — CMS synthetic data (SynPUF-derived) represents general Medicare claims. Skin substitutes are a niche category (~$10B in real Medicare but absent from synthetic sets.

Revised approach for #234:

  1. Build the extraction query for BOTH product codes (Q4xxx) AND application codes (15271-15278)
  2. Generate synthetic skin sub claims based on known utilization patterns (OIG report figures) for development/testing
  3. The real analysis will use actual CCLF data when available
  4. The synthetic generator needs to produce realistic patterns: provider clustering, setting distribution, geographic variation, appropriate diagnosis pairing (L97.x chronic ulcer codes)

Key methodological note: On a real Medicare claim, a skin substitute encounter typically has:

  • Line 1: Q4xxx (product) — billed units = cm² applied
  • Line 2: 15271/15275 (initial application) — 1 unit
  • Line 3: 15272/15276 (additional application add-on) — if >25cm²
  • Diagnosis: L97.xxx (non-pressure chronic ulcer) or L89.xxx (pressure ulcer)
  • Place of service: 11 (office), 22 (HOPD), 24 (ASC)
  • Rendering NPI: podiatrist, dermatologist, or wound care NP

This claim structure is what the predatory pattern detection (#242) will analyze.

Progress: Synthetic claims generated, loaded into DuckDB.

Completed:

  • Built synthetic claim generator (dev/scripts/generate_skin_sub_claims.py)
  • Generates BOTH product codes (Q4xxx) AND application codes (15271-15278) per encounter
  • Realistic claim structure: 2-3 lines per encounter (product + initial application + add-on if >25cm²)
  • Distributions sourced from OIG Sept 2025 report and CMS CY 2026 OPPS Final Rule:
    • Provider specialty: 60% podiatry, 15% derm, 10% surgery, 10% NP, 5% other
    • Setting: 70% office, 15% HOPD, 10% ASC, 5% SNF
    • Geography: Sun Belt weighted (FL 18%, TX 14%, CA 12%)
    • Products: top 10 by utilization volume (EpiFix, Grafix, Apligraf, etc.)
    • Wound size: tiered by provider volume (normal 5-25cm², high 10-50cm², extreme 20-100cm²)
    • Diagnoses: L97.x chronic ulcers, L89.x pressure ulcers, E11.621 diabetic foot ulcer
  • Provider volume tiers: 85% normal, 10% high, 5% extreme (for predatory pattern testing)
  • Loaded as skin_subs.claims_synthetic (4,257 lines, 2,000 encounters)
  • Every row has source: synthetic and source_methodology for provenance

Results:

  • 2,000 encounters → 4,257 claim lines (2,000 product + 2,000 application + 257 add-on)
  • 860 unique patients, 200 providers, 14 HCPCS codes
  • $2.3M total paid across 3 years (2022-2024)

Remaining:

  • Filter real CCLF data when available
  • Link to eligibility for beneficiary demographics
  • Link to provider NPI data
  • Compute utilization metrics

Analytical tables built in DuckDB. Providers (200), beneficiaries (1,255), utilization_summary (199), products_enriched (286), manufacturers (68), asp_trajectories (1,974). Podiatry 60% of claims, office setting 68% of spend, 39 products with anomalous spikes, 105 products lose under flat rate.
