P49: Code families as first-class objects — element model, lineage anchors, longitudinal chat

Spec docs/superpowers/specs/2026-09-09-code-family-longitudinal-design.md. A code is a bundle of logical elements (CMS's telehealth Step 3: "Review the elements of the service as described by the HCPCS code", 90 FR 32389). P49 makes elements, families and lineage events tables with FR paragraph anchors, stamps code/family anchors across rules, comments, IOM, eCFR and MLN, and teaches the chat to build a cited longitudinal view of any code payable under the PFS. P50 joins that timeline to outcome series (legal epidemiology).
