The RFI response should show CMS that palliative-care payment concerns are a recurring commenter theme, not a new ask. We hold 166,941 PFS-rulemaking comments in bib across ten dockets (CMS-2017-0092 … CMS-2026-2377, CY2018–CY2027), with local full text for ~23.8k (combined.md under .state/comments/) and abstracts for ~6.4k more. A lexical scan finds ~270 comments whose retrievable text references palliative care (stem palliativ), concentrated in CY2019 (71) and CY2024 (44).

Plan:

  1. dev/scripts/tag_palliative_comments.py — sweep every comment item (combined.md body, else abstract), tag hits topic:palliative in bib, record mention count + sentence snippets in the item's extra_json, and store per-docket sweep stats (total / with-text / hits) in bib_meta so the notebook can disclose denominators without needing .state.
  2. Run the sweep; scoped Zotero sync (--tag topic:palliative) so the tagged comments are citable from the evidence library.
  3. New section in notebooks/palliative_care_rfi.py: per-rule-year mention counts with text-coverage denominators, a table of the referencing comments (org, year, mentions, regulations.gov link), and a thematic summary with citations.

Related: P35 #574-#576 (LLM closed-vocab tagging write-back) is the general mechanism, still open — this is a narrow lexical pass that doesn't wait for it.

Done (f7fac3f, 4bfe624):

  • 278 comments tagged topic:palliative across the ten PFS dockets, in bib and synced to Zotero (scoped --tag). Per docket year: 2017: 13 · 2018: 78 · 2019: 31 · 2020: 7 · 2021: 24 · 2022: 18 · 2023: 44 · 2024: 33 · 2025: 28 · 2026: 2. The 2018 spike is the CY2019 E/M code-collapse response from HPM clinicians.
  • Sweep: dev/scripts/tag_palliative_comments.py (idempotent; mention counts + sentence snippets in extra_json; per-docket denominators in bib_meta.palliative_sweep). Coverage disclosure: 28,416 of 166,941 docketed comments have locally retrievable text.
  • Notebook: new Section 4 "Prior Rulemaking Comment Record" in palliative_care_rfi.py — per-year chart, live table of all 278 with regulations.gov links, and a five-theme decade summary (AAHPM in all nine prior cycles; payment adequacy; team-based delivery; MIPS #495/NCP/NASW quality thread; telehealth/preventive access framing). Headless nb_integration pass: green.

Re-run the sweep after #615 (post-close re-farm) or any new attachment extractions.
