P39: CY2027 Notebook Deep-Dive — GPCI locality analysis + primary-care (ACO attribution) services

Close the CY2027 NPRM notebook's two confessed gaps: (1) GPCI — load NPRM Addendum E into pfs.gpci_proposed (CMS-1832-P + CMS-1848-P partitions) and add a locality analysis section (near-term proposed-vs-final deltas, long-term 2016–2027 trends, locality-adjusted payment); (2) primary-care services — publish the 109-code MSSP attribution list (42 CFR 425.400(c)) to the lake and analyze every code individually, near-term (CY2026 final vs CY2027 proposed) and long-term (2015–2027, RVU and dollar terms).
