bib: fix Rule citation to use proper FR format (Volume FR Page) #301

Current _format_rule produces:
Centers for Medicare & Medicaid Services. (2025). CY 2026 PFS Final Rule. Federal Register, 90, 98452.

Bluebook requires:
Medicare and Medicaid Programs; CY 2026 Payment Policies Under the Physician Fee Schedule, 90 Fed. Reg. 101174 (Dec. 31, 2025).

Need: FR volume, first page, full date, proper title abbreviation. The data is already in the Rule model (fr_volume, fr_page, date_published).
