Generate and execute CREATE NAMESPACE / CREATE TABLE statements:

Walk all SQLTable subclasses, group by schema
Generate Iceberg DDL using SQLTable.to_ddl() adapted for Iceberg
Deploy via Nessie catalog REST API or Trino connection
Idempotent: skip existing tables, alter for new columns
Add [lake] section details to stack.toml if needed
